Join the South Warwickshire University NHS Foundation Trust as a District Nurse Clinical Lead, where you will have the opportunity to lead, inspire, and shape the future of community nursing. This role is based in Stratford Upon Avon, Shipston on Stour, and the surrounding villages, and is focused on the assessment and management of complex health needs within the community.

The District Nurse Clinical Lead will be responsible for developing, implementing, and evaluating care programs while working both autonomously and collaboratively within a Place-Based Team. Your advanced clinical assessment and prescribing skills will be essential in delivering high-quality, person-centered care.

Main Responsibilities

• Lead the assessment and management of complex health needs within the community nursing team.
• Develop, implement, and evaluate care programs while working both autonomously and collaboratively within a Place-Based Team.
• Provide advanced clinical assessment and prescribing to ensure high-quality, person-centered care.
• Collaborate with the Professional Lead to align team operations with service goals and standards.
• Manage a diverse caseload across urban and rural settings, ensuring effective care delivery.
• Supervise and develop the competencies of nursing staff, including conducting performance reviews and clinical supervision.
• Act as a resource for the team, providing education and support to enhance clinical practice.
• Engage in regular audits and monitoring of care delivery to ensure compliance with quality standards.
• Communicate complex and sensitive information effectively to patients, families, and team members.
• Participate in the recruitment and selection process for new staff.

Requirements

• Must be a Registered Nurse with significant community experience managing patients with complex needs.
• Completion of the District Nurse Specialist Practitioner Qualification (DNSPQ) is required.
• Must hold a V300 independent prescribing qualification.
• Demonstrated ability to make high-level clinical decisions and adapt to various clinical circumstances.
• Experience in leading clinical teams and managing patient care autonomously.
