Nurse Practitioner Overview

The Adult Community Nursing service’s Single Point of Access (SPA) team is seeking a dedicated Nurse Practitioner to join its Clinical Assessment Team (CAT). This position is ideal for nurses who are passionate about providing high-quality care and are committed to excellence. The CAT Team operates within the Adult Community Nursing Service at Homerton Healthcare, collaborating with District Nursing Neighbourhood Teams and Specialist Services, including Tissue Viability and Continence nursing teams. The team serves the City and Hackney areas, focusing on holistic care assessments and implementing care plans for patients in their homes, as well as preventing hospital admissions by managing acutely unwell patients at home.

The successful candidate will closely collaborate with the Senior Nurse Practitioner and the multidisciplinary team, demonstrating excellent communication skills. They will leverage their clinical knowledge and evidence-based practices to independently conduct assessments and provide care in patients’ homes. Candidates should be motivated, compassionate, well-organized, and eager to enhance their clinical skills and knowledge.

Main Duties

• Utilize clinical expertise to foster patient independence and facilitate self-care.
• Provide short-term nursing interventions and refer patients to appropriate neighborhood services for long-term care.
• Conduct initial holistic assessments for referred individuals and implement tailored care plans.
• Exhibit professional leadership and mentor junior staff to enhance their clinical skills.
• Collaborate within a multidisciplinary team to assess, plan, implement, and evaluate patient care needs.
• Deliver high-quality holistic nursing care while promoting health education and patient autonomy.
• Ensure the assessment and management of current and anticipatory care needs, coordinating with neighborhood services.
