An exciting opportunity has arisen for a District Nurse Caseload Holder within the Bethesda area, specifically with the Bangor/Bethesda District Nursing Team. This role is designed for individuals who hold the District Nurse Specialist Practitioner Qualification, are working towards it, or are willing to start the course at the next intake.

The successful candidate will be responsible for managing a designated caseload of patients, ensuring the delivery of high-quality nursing care in line with established best practices. This position involves conducting complex needs assessments, including Continuing Healthcare (CHC) and Health and Social Care evaluations, while providing specialist district nursing services. The role requires collaboration with various agencies without direct supervision, and the post holder will also undertake management duties and act as a deputy in the absence of the Team Leader.

A commitment to continuous professional development is essential, along with community nursing experience at Band 5 or equivalent, particularly with patients who have acute or complex needs.
