Urgent Care Practitioner Overview

We are looking for dynamic Band 6 practitioners to join our team as Urgent Care Practitioners within the Crisis Resolution Home Treatment Team (CRHTT). This role is open to qualified Nurses, Social Workers, and Occupational Therapists who are passionate about delivering high-quality mental health services.

The successful candidates will work closely with acute mental health hospitals and community teams to support individuals experiencing a mental health crisis. Our CRHTT provides the benefits of hospital admission while allowing patients to remain at home, including home visits from our experienced multidisciplinary team. We are committed to a focused recovery model and require practitioners to work on a rota basis, covering 7 days a week throughout the year.

Main Responsibilities

• Conduct inclusive and empathetic assessments for service users accessing CRHT services.
• Utilize clinical skills to assess individual needs and risks effectively.
• Develop and implement care plans that include education and supportive discharge planning.
• Collaborate with families, carers, and partner agencies to ensure the safety and well-being of service users.
• Participate in multi-agency meetings and conduct supportive discharge assessments.
• Contribute to the development of high-quality mental health services.

Qualifications and Experience

• Registered Nurse with current registration in mental health, learning disabilities, or adult nursing.
• Completion of a mentorship course or equivalent qualification.
• Evidence of continuous professional development.
• Significant post-qualification experience in the relevant care group.
• Experience working with individuals facing complex health issues.
• Ability to supervise staff and provide clinical interventions.
• Experience in conducting clinical and risk assessments.
• Familiarity with partnership working across various teams and agencies.
• Understanding of clinical governance and experience within the CPA process.
• Previous community experience is preferred.
