An exciting opportunity has arisen for a mental health nurse to join the Memory Assessment Service (MAS) as a Band 6 Memory Assessment Nurse. This role involves providing modern and supportive holistic care during the initial period of cognitive assessment and diagnosis, working across both Wrexham and Flintshire sites.
The successful candidate will be responsible for working with individuals experiencing cognitive changes and concerns regarding dementia. This includes conducting initial comprehensive assessments, delivering diagnoses, and formulating treatment plans. You will manage a caseload of patients and participate as a full member of the multi-disciplinary team, collaborating with colleagues to ensure effective patient care.
In this role, you will lead the MAS in the absence of the manager and will need to work autonomously in both clinic and community settings, conducting home visits as necessary. The ability to make complex decisions independently is crucial, especially when working in isolated areas with limited transport links.
We are looking for candidates who meet the essential qualifications and experience criteria outlined in the job description. Knowledge of organic mental health issues is essential, and the ability to communicate in Welsh is desirable.
This position offers a competitive salary ranging from £40,559 to £48,841 per annum and is a permanent full-time role. We are committed to supporting your career development and providing opportunities for progression within our organization.
