Join the South Warwickshire University NHS Foundation Trust as a District Nurse Clinical Lead and take on a pivotal role in shaping the future of community nursing. This position is based in Stratford Upon Avon, Shipston on Stour, and surrounding areas, where you will be responsible for assessing and managing complex health needs within the community.

Your primary responsibilities will include leading the development, implementation, and evaluation of care programs, working both autonomously and collaboratively within a Place-Based Team. Your advanced clinical assessment and prescribing skills will be crucial in delivering high-quality, person-centered care.

Main Duties

• Lead the assessment and management of complex health needs for a designated population.
• Develop, implement, and evaluate care programs in collaboration with a multidisciplinary team.
• Provide clinical leadership and ensure high standards of care for patients on the community nursing caseload.
• Conduct advanced clinical assessments, including diagnosis, prescribing, and planning of care.
• Supervise and support the development of nursing staff competencies, including performance reviews and recruitment.
• Work autonomously while also collaborating with the Professional Lead to align with service goals.
• Engage with patients and their families to create personalized care plans based on comprehensive assessments.
• Utilize advanced communication skills to manage sensitive information and support team members.
• Promote evidence-based practice and quality improvements within the team.
• Participate in clinical audits and monitor care delivery to ensure compliance with standards.

Requirements

• Must be a Registered Nurse with significant community nursing experience.
• Hold the District Nurse Specialist Practitioner Qualification (DNSPQ) and be a V300 independent prescriber.
• Demonstrate advanced clinical assessment skills and the ability to manage complex health conditions.
• Possess strong leadership skills and the ability to work collaboratively within a multidisciplinary team.
• Have experience in developing and evaluating care plans and interventions for patients with complex needs.
• Ability to communicate effectively and manage challenging situations with patients and team members.
