Children Community Learning Disability Nurse Overview

This position is for a Children Community Learning Disability Nurse within the Learning Disability Care Hub, specifically as a member of the Children’s Learning Disability Team in Oldham. The successful candidate will be responsible for providing specialized health and behavioral assessments to children and young people with learning disabilities who are referred to the team. Interventions will be conducted in various environments, including the child’s home, necessitating collaboration with partners and stakeholders to ensure the delivery of responsive, effective, safe, and child-centered services.

Key Responsibilities

• Collaborate as part of the Children’s Learning Disability Team within the Learning Disability Care Hub in Oldham.
• Conduct specialized health and behavioral assessments for children and young people with learning disabilities referred to the team.
• Deliver interventions in various settings, including the child’s home.
• Work closely with partners and stakeholders to provide effective, safe, and child-centered services.
• Adhere to the policies and procedures of Pennine Care NHS Foundation Trust at all times.
• Provide timely and efficient community nursing services to children, young people, their families, and caregivers.
• Perform comprehensive assessments for children with learning disabilities and complex needs, requiring specialized knowledge.
• Participate in additional training for implementing high-quality Positive Behavioral Support.
• Offer behavior support training to relevant teams surrounding the child.
• Develop specialized care plans and interventions.
• Promote health and well-being while aiming to prevent ill health.
• Assist community learning disability nurses in addressing health inequalities faced by individuals with learning disabilities.
• Collaborate with primary and secondary care services to meet the health needs of individuals with learning disabilities.
• Actively identify and manage risks.
• Work in partnership with Social Services and Child Safeguarding.

Requirements

• Must be a Registered Nurse in Learning Disabilities (RNLD).
• Post-registration training or willingness to undertake training, along with experience equivalent to a postgraduate diploma level.
• Demonstrated knowledge and experience in supporting individuals with learning disabilities.
• Experience in providing advice and support to a diverse range of individuals.
• Proven experience in partnership working and risk management.
• Desirable: Experience in delivering training and previous community nursing experience.
• Essential knowledge of person-centered approaches and LD specialist assessment tools.
• Familiarity with behavioral techniques and management.
• Ability to handle complex and sensitive situations, including challenging behavior and safeguarding issues.
• Knowledge of current strategic drivers for community learning disability teams.
• Desirable: Understanding of health issues affecting children and young people with learning disabilities, as well as knowledge of Transforming Care and SEND/EHC.
