We are seeking a dedicated Learning from Deaths Clinical Specialist to join our team at The Royal Shrewsbury Hospital. This role is ideal for individuals passionate about quality improvement and enhancing patient care. The successful candidate will play a crucial role in promoting the Learning from Deaths agenda across the organization, supporting the mortality review process, and engaging with various stakeholders.
Main Responsibilities
- Provide leadership on all aspects of Learning from Deaths within the Trust, facilitating improvements in clinical practice and safety.
- Promote a positive learning environment to enhance patient safety, experience, and outcomes.
- Foster an open learning culture that identifies and shares lessons learned from both positive and negative practices.
- Disseminate learning outcomes and provide intelligence to support quality improvement initiatives.
- Maintain and develop the Learning from Deaths program in accordance with local and national standards.
- Ensure proper triangulation of cases, including HMC referrals, formal complaints, and reviews under the Patient Safety Incident Response Framework (PSIRF).
- Identify and communicate lessons learned from the Learning from Deaths processes to relevant stakeholders and participate in various governance forums across the Trust.
Qualifications and Experience
- Must be a registered healthcare practitioner, such as an NMC Registered Nurse or Midwife, AHP, HCPC registered Paramedic, or an equivalent registered clinician.
- Educated to degree level or possess equivalent experience.
- Experience working in a complex and changing environment with the ability to prioritize effectively.
- Knowledge of NHS policies and initiatives related to Learning from Deaths.
- Desirable: Familiarity with the Structured Judgment Review methodology for conducting reviews.
- Desirable: Understanding of Quality Governance and Risk Management systems.
- Excellent written communication skills for producing various types of documents to meet delivery needs.
- Ability to analyze complex issues and develop practical solutions.
