The Medicine for Older People Department at East Lancashire Hospitals NHS Trust is seeking a highly skilled and motivated Advanced Clinical Practitioner specializing in Frailty. This role presents an exciting opportunity for a qualified ACP to join a growing and dynamic team dedicated to enhancing frailty services. The department is led by two Consultant ACPs, supported by Consultant Geriatricians, Specialty Doctors, Junior Clinical Fellows, nursing leaders, AHPs, and a comprehensive multi-disciplinary team (MDT). Over the past year, significant advancements have been made in frailty services, resulting in a high-performing seven-day service. We are looking for an enthusiastic ACP to contribute to our ongoing improvements.

Main Responsibilities

• Conduct clinical and holistic assessments of frail elderly patients in the Older Persons Rapid Assessment Unit and other clinical areas as necessary.
• Provide expert guidance and advice to staff to ensure safe and effective care for frail elderly patients.
• Act as a clinical leader within the trust.
• Support and guide the ongoing development of frailty services across ELHT.
• Collaborate closely with the wider multi-disciplinary team to ensure high standards of care.

Requirements

• Registered healthcare professional with current registration (NMC or HCPC).
• Completion of a Master’s level ACP program.
• A minimum of five years post-registration experience in a relevant setting.
• Evidence of continuing professional development.
• Independent and Supplementary Prescribing qualifications.
• Advanced history taking and patient assessment skills or equivalent experience.
• Experience in leading quality improvement projects.
• Understanding of Elderly Medicine/Frailty principles and the ACP role.
• Strong communication and interpersonal skills.
• Ability to manage a caseload independently.
• Proficiency in using electronic health records.
• Critical thinking and problem-solving skills.
• Awareness of current healthcare issues and developments.
• Compassionate and patient-centered approach.
• High level of professionalism and integrity.
• Commitment to personal and professional development.
• Willingness to adapt to changing service needs.
• Able to manage priorities under pressure.
• Flexibility to work extra hours and rotate between clinical areas.
