Join the South Warwickshire University NHS Foundation Trust as a District Nurse Clinical Lead and take on a pivotal role in shaping the future of community nursing. This position is based in Stratford Upon Avon, Shipston on Stour, and the surrounding areas, where you will be responsible for assessing and managing complex health needs within the community.

Your primary duties will include leading the development, implementation, and evaluation of care programs, working both autonomously and collaboratively within a Place-Based Team. Your advanced clinical assessment and prescribing skills will be essential in delivering high-quality, person-centered care.

Main Responsibilities

  • Lead the assessment and management of complex health needs in the community.
  • Develop, implement, and evaluate care programs.
  • Provide clinical leadership for the community nursing caseload.
  • Conduct advanced clinical assessments and prescribe medications.
  • Collaborate with multidisciplinary teams for comprehensive patient care.
  • Supervise and develop nursing staff competencies.
  • Engage in audits and monitoring of care delivery.
  • Act as a resource for team education and support.
  • Communicate effectively with patients and families regarding care plans.
  • Participate in recruitment and selection of new staff.

Requirements

  • Registered Nurse with substantial community experience managing complex patients.
  • Completion of the District Nurse Specialist Practitioner Qualification (DNSPQ).
  • V300 independent prescribing qualification.
  • Ability to make high-level clinical decisions.
  • Experience in leading a community nursing team.
  • Strong communication skills for handling sensitive information.
