One Medical Group is seeking an experienced Advanced Clinical Practitioner to join our multi-disciplinary team at Rosedean House Surgery. This role is pivotal in transforming and modernizing care pathways, ensuring effective sharing of skills across traditional professional boundaries. The Advanced Clinical Practitioner will be responsible for making autonomous decisions regarding the assessment, diagnosis, and treatment of patients. You will work closely with a diverse team, including GPs, practice nurses, wellbeing advisors, health coaches, and physician associates. Your role will also involve contributing to the clinical governance of the practice in collaboration with the lead nurse, ensuring that we deliver safe and compliant services to all patients.
Key Responsibilities:
- Make independent decisions in patient assessment, diagnosis, and treatment.
- Collaborate with a multi-disciplinary team to enhance patient care.
- Contribute to clinical governance alongside the lead nurse.
- Provide clinical leadership to ensure effective service delivery.
- Manage complete episodes of patient care while adapting to new healthcare delivery methods.
Requirements:
- Current registration with NMC, HCPC, or GPhC.
- MSc in Advanced Practice or relevant Level 7 ACP courses, including modules in Advanced Physical Assessment Skills, Minor Injuries, and Disease Management.
- Certification as an Independent Prescriber.
- Experience in primary care and minor illness management.
- Strong communication skills and ability to perform under pressure.
- Demonstrated clinical reasoning and decision-making skills.
- Ability to inspire and lead a team.
