Join the Cwm Taf Morgannwg University Health Board as a Band 6 Occupational Therapist Specialist in our Rapid Assessment and Prevention OT service. This role offers a unique opportunity to work collaboratively with GP and Welsh Ambulance colleagues to intervene at the earliest point of crisis, helping individuals avoid unnecessary trips to Accident & Emergency.

The primary focus of this position is to conduct rapid multi-factorial assessments in patients’ homes, identifying their needs and co-producing achievable goals. You will implement fast-paced interventions aimed at preventing hospital admissions and supporting individuals in managing their immediate needs. Collaboration with integrated Health and Local Authority community teams is essential to deliver care based on a place-based care ethos.

In this role, you will prioritize and screen referrals to provide same-day responses, which may include telephone consultations and home assessments. You will also work closely with primary care and rehabilitation colleagues to ensure effective handover of care episodes. As part of the service design, you will work with senior occupational therapy staff to implement changes swiftly and engage in service evaluation to contribute to the future design of community services.

Qualifications and Experience

To be considered for this role, you must possess a Degree or Diploma in Occupational Therapy and have postgraduate training or accreditation in fieldwork education. A solid understanding of clinical governance principles and risk management is essential, along with knowledge of professional ethics and standards. You should have post-graduate clinical experience in physical medicine and service delivery across various clinical settings.

Skills and Abilities

Strong interpersonal and communication skills are crucial, as is the ability to work effectively under pressure within a multidisciplinary team. You should demonstrate excellent organizational skills for managing caseloads, including time management and prioritization. The ability to work independently and make effective clinical decisions is also required.
