One Medical Group is seeking an experienced Advanced Clinical Practitioner to join the team at Rosedean House Surgery. This role is pivotal in transforming and modernizing care pathways, ensuring effective collaboration across professional boundaries. The practice has recently received a ‘Good’ rating from the CQC and is dedicated to continuous improvement in service delivery.
The Advanced Clinical Practitioner will be responsible for making autonomous decisions regarding patient assessment, diagnosis, and treatment. This position requires a strong commitment to high standards of patient care and the ability to inspire colleagues. The successful candidate will work closely with a multi-disciplinary team, including GPs, practice nurses, wellbeing advisors, health coaches, and physician associates.
Key Responsibilities:
- Make independent decisions regarding the assessment, diagnosis, and treatment of patients registered at the practice.
- Provide clinical leadership in collaboration with the lead nurse to ensure the safe, effective, and compliant delivery of healthcare services.
- Work alongside a multi-disciplinary team, including GPs, practice nurses, wellbeing advisors, health coaches, and physician associates.
- Manage whole episodes of care while contributing to the overall healthcare delivery within the team.
- Inspire and lead the team by demonstrating high standards of patient care and a commitment to professional development.
Requirements:
- Current registration with NMC, HCPC, or GPhC.
- Master’s degree in Advanced Practice or relevant Level 7 ACP courses, including recognized modules in Advanced Physical Assessment Skills, Minor Injuries, and Disease Management.
- Certification as an Independent Prescriber.
- Experience in primary care and minor illness management.
- Strong communication skills and the ability to perform under stress.
- Demonstrated flexibility and a willingness to adapt to new ways of working.

This position is available on a full-time basis (40 hours per week) with part-time options also considered. The role is located at Rosedean House Surgery, 8 Dean St, Liskeard PL14 4AQ.
