- Go to File > New from Template.
 - In Search, type form.
 - Double-click the template you want to use.
 - Select File > Save As, and pick a location to save the form.
 - In Save As, type a file name and then select Save.

				Enable the Developer Tab: Go to “File” > “Options” > “Customize Ribbon,” check “Developer,” and click “OK.”
Open the Developer Tab: Click on the “Developer” tab on the Word ribbon.
Insert Content Controls: Use Content Controls (e.g., text boxes, dropdown lists) from the Developer tab to create the form structure.
Protect the Document: Go to “Developer” > “Protect Document” > “Restrict Editing.”
Select User Entry Areas: In the “Restrict Editing” pane, mark the areas users can edit.
Save and Distribute: Save the form and distribute it to users for data entry.
